Conventional jaw reduction makes a visible difference for people with a wide jaw and a small chin. For people whose whole jaw is broad, with both a wide jaw and a well-developed chin, the change can be hard to see.

So that the change also shows from the front, we add chin-narrowing surgery to jaw reduction. We reduce both jaw angles and slim the chin at the same time, joining the side and front jawlines into a small V-line.

Because the focus is on making the whole face look smaller, it also suits people whose square look comes mainly from a wide chin.

Asymmetry can be corrected at the same time. We also perform revisions for people who had jaw reduction elsewhere but saw little change.

Results vary with your bone structure and constitution.

Aftercare

This surgery reshapes bone, so how you spend the time until the swelling settles and the bone heals matters.

  • Swelling and bruising appear for about 2 days after surgery, then gradually fade over about 2 weeks.
  • For 3 days after surgery, cool the area often to limit swelling (repeat 15 minutes of cooling and 5 minutes of rest). From day 4, switch to warm packs to help the swelling go down faster.
  • Keeping your upper body raised, with the treated area above your heart, helps the swelling go down faster.
  • For 1 week after surgery, eat soft foods or liquids such as rice porridge or thin rice gruel, and avoid hard foods for 4 weeks.
  • If the surgery was done through the mouth, rinse your mouth about 4 times a day to keep the incision clean.
  • For 3 weeks after surgery, avoid alcohol and smoking, and rest without overdoing things.
  • For 4 weeks, until the cut bone has firmly healed, take care not to bump your face or fall. Sleep on your back so neither side of the jaw is pressed.

Risks and side effects

Results and recovery vary from person to person. The main risks and side effects are:

Methods

Bone, muscle and fat, treated to suit your jaw

A wide-looking jaw is not always caused by bone alone. Developed muscle or fat can also play a part, so we choose from the methods below to suit your jaw.

Reshaping the bone

  • Mandibular angle resection

    The lower jaw, from below the ear to near the mouth, is trimmed in a long, gentle curve.

    It turns a square, angular lower jaw, as seen from the front, into a softer contour. Marked asymmetry can also be corrected.

  • Cortical bone resection

    Bone has three layers: the hard outer cortical bone, the spongy cancellous bone inside, and the inner cortical bone. Cortical bone resection carefully removes a single layer of the outermost cortical bone to reduce fullness in the face.

    Because the outer bone is shaved off like peeling a banana, the change tends to show from the front. If muscle and fat are also developed, removing them at the same time can be expected to give a greater change.

Reducing muscle and fat

  • Muscle reduction

    A radiofrequency current is passed through a medical needle to burn and shrink part of the muscle.

    When the masseter, the chewing muscle, is developed and thick, shrinking it makes the contour look slimmer. Muscle reduced this way tends not to grow back, so the change tends to last.

  • Buccal fat removal

    The side of the face has a pad of fat (the buccal fat pad) running from the temple to beside the lips. When this fat grows large around the cheeks and makes them look full, it is called buccal fat.

    It usually shrinks from childhood into adulthood, but it can remain in adults.

    The surgery is done through the mouth. Removing too much can damage nearby nerves or salivary glands, or leave the cheeks hollow. We confirm the exact position and remove about 4–5 cc of fat.
