Short Chin Surgery

Bringing a set-back chin forward for a cleaner profile

When the chin tip has not fully developed and sits back, the chin can look short and the mouth can look like it sticks out.

Short chin surgery adjusts the length and position of the chin, aiming for a cleaner profile. Methods include an implant, chin bone advancement, fat transfer and others.

Who it is for

  • Your bite is normal, but your chin is long and sticks forward
  • Your bite is normal, but your chin did not fully develop and is small
  • The line between your neck and face is unclear
  • The lines around your mouth are deep and noticeable

What is a short chin?

The chin did not grow enough, so it sits back and looks short.

When the chin is set back or too short, the balance of the whole face looks off. The mouth can look like it sticks out, which may give a stubborn or vague impression.

In these cases, it is corrected with an implant, chin advancement or a graft of your own fat.

A guide to profile balance

Seen from the side, draw a line from the nose tip to the chin. An angle of about 75–85° between this line and a horizontal line is one guide.

This number is only a guide. The actual design is decided to suit your own features.

Implant vs. bone surgery

An implant changes the jawline relatively little, while chin advancement, which cuts and moves the bone, changes it a lot.

If your short chin is not severe, an implant is a good fit. Dr. Cho Bae-jung examines you at the consultation and decides which method suits you.

Results vary with your bone structure and constitution.

Aftercare

To help swelling settle quickly and the incision heal cleanly, please follow these points.

  • Start using cold packs right after surgery and keep using them until the swelling settles. This helps it go down.
  • Do not eat for 1 day after surgery. For the next 7 days, have soft liquid foods such as thin rice porridge, juice and milk.
  • From 2 weeks, you can eat almost normally. Cut food into small pieces, and wait until 4 weeks before eating anything hard.
  • For 3–4 days after surgery, wear a compression band on your face. When sleeping, do not lower your head or lie face down. Supporting your head on 2–3 soft pillows eases pain.
  • To prevent inflammation and infection inside the mouth, rinse your mouth after every meal and keep it clean.
  • Alcohol and smoking slow wound healing. Avoid them for at least 3–4 weeks after surgery.
  • During the downtime, take care not to let your chin take a hard knock.

Risks and side effects

Results and recovery vary from person to person. The main risks and side effects are:

  • Swelling and bruising after surgery or treatment
  • Infection or inflammation
  • Asymmetry, or a result that differs from what you pictured
  • Scarring (when an incision is made)
  • Numbness or changes in sensation (often temporary)

Methods

Implant, bone or fat: the method that fits your chin

Methods with an incision

  • Implant insertion

If your short chin is not severe, an implant is placed in the chin to make it larger or change its position.

An opening of about 2–3 cm is made in the mucosa at the chin, from inside the mouth. The periosteum is then lifted to make a pocket for the implant. The implant is placed and fixed, and the incision is closed.

  • Chin advancement

The chin bone is cut and moved forward. The angle of the cut can also adjust the length of the chin, for a natural chin shape.

An incision of about 2–3 cm is made at the chin and the periosteum is lifted to expose the bone. The bone is cut along the planned line, and the lower piece is brought forward as far as planned, fixed and closed.

Methods without an incision

  • Fat transfer

Excess fat is taken from the thighs, abdomen or another area, and only the fat suited to grafting is separated out.

That fat is grafted into the chin in several layers by injection, without an incision.

    Fat Transfer
  • Hyaluronic acid

This is for people who cannot take downtime, or who feel uneasy about surgery and want a simpler treatment. Hyaluronic acid is injected into the short chin to shape the jawline.

If the chin muscles are strong, chin Botox may be done at the same time when needed.
