As you age, the membrane that supports the fat gets weaker, causing fat to move downward. This procedure is a relatively simple surgery that removes fat under the eyes through an incision.

Lower eyelid fat removal

As you age, the strength of the membrane supporting the fat weakens, causing the fat to descend and accumulate under the eyes. This surgery involves relatively simple removal of the fat through the conjunctiva.

Symptoms may worsen under stress or when health deteriorates, and it can even occur from the 20s. Under-eye fat can contribute to dark circles and a tired appearance, so removing it can create a brighter impression.

With age, the membrane surrounding the lower eyelid fat weakens, allowing the fat within to migrate downwards, creating a protrusion under the eyes.

The accumulation of fat is a sign of aging, and lower eyelid fat can be corrected surgically in procedures performed by cosmetic surgeons. Recently, dermatologists have also been using lasers to remove some of the fat through the conjunctiva.

Laser removal of under-eye fat is bloodless and typically does not result in bruising. This procedure permanently removes the protruding fat and is not prone to recurrence. In some cases, where fat distribution is uneven, surgeons may also redistribute or transplant fat to areas with insufficient volume.

Surgical Procedure

1. Identify the position of the excessively enlarged fat under the eyes.
2. Use a laser to make an incision in a part of the lower eyelid conjunctiva and expose the fat.
3. Cut the protruding fat that comes out when the skin under the eyes is gently pressed.
4. After hemostasis, suture with approximately one or two points.

Under Eye Bags

The under eye bags surgery refer to a surgery that deliberately creates protrusions under the eyes when smiling.

Insufficient swelling under the eyes can make the face look less voluminous and older. However, having under eye bags not only gives a youthful and cute impression but also provides a bright and refreshing look when smiling.

Filler (Hyaluronic Acid)

A substance injected under the skin or subcutaneous fat to correct concave areas or provide volume is called filler (hyaluronic acid).

Hydroxyapatite, which is the same natural component as hyaluronic acid, is injected into the lower eye area to create tear bags. It reduces fine lines under the lower eyelids and emphasizes the lower eye area, giving a younger and more vibrant appearance. Since the effect of fillers (hyaluronic acid) lasts for about a year, regular retreatment is necessary.

– Procedure: Apply an anesthetic cream to the lower eye area and wait for about 40 minutes before injecting the filler (hyaluronic acid) into the tear bag area.

Autologous Fat Transplantation

This is a method of extracting unnecessary fat from areas such as the thighs, buttocks, and abdomen, and transplanting it under the eyes. It allows for body contouring simultaneously since excess fat is harvested.

1. Local anesthesia and sedation anesthesia are administered concurrently.
2. For fat grafting, fat is harvested using thin tubes from areas such as the thighs, buttocks, and abdomen.
3. The harvested fat is processed through a centrifuge to remove impurities and isolate pure fat cells.
4. The purified fat is evenly inserted into various layers under the eyes.

Under-eye wrinkle removal

The skin around the eyes is very thin, making it prone to wrinkles and sagging. The most important aspect of treating wrinkles around the eyes is achieving natural results. A precise analysis of the factors contributing to the appearance of aging is conducted, followed by determining the appropriate treatment method and surgical procedure. For less severe wrinkles, non-surgical methods may be temporarily applied.

In cases of advanced aging, surgical intervention may be necessary. During surgery, it is crucial to minimize visible scarring and maintain a natural appearance.

Non-surgical methods to improve under-eye wrinkles include Botox and fillers (hyaluronic acid), while surgical options include lower eyelid surgery. Initially, there may be some redness as the scars mature, but over time, the scars typically become less noticeable. The surgery typically lasts around an hour, and stitches are removed around the fifth day post-surgery, after which normal activities can resume.

Postoperative Care Instructions

1. After the surgery, it is advisable to continue using cold compresses until the swelling subsides.
2. There may be variations in swelling and bruising after the surgery. Avoid sleeping face down or on your side, and using soft pillows (or cushions) stacked two to three high can help reduce swelling and discomfort.
3. Avoid rubbing or touching the eyes excessively.
4. Avoid alcohol and tobacco for 3-4 weeks after the surgery, as it may slow down the healing of the scars.
5. You can start wearing contact lenses after 2 weeks. During the first 2 weeks, wear glasses or sunglasses.
6. You can start wearing light makeup after 2 weeks. Eyelash extensions can be applied approximately 1 month later.
7. Avoid taking baths or going to saunas for approximately 3 weeks.
