Revision Eye Surgery

Finding what went wrong, then reshaping the crease

A crease that is too wide, eyes that do not match, a crease that has come undone. Revision eye surgery is for when you are unhappy with an earlier eye surgery, or want to correct the line and shape of your crease.

In a revision, it matters to pinpoint the cause and choose a method that fits it. At KOKO Plastic Surgery, we check the state of your previous surgery first, then reshape your eyes the way you want.

Who it is for

  • Your crease is gradually coming undone
  • Your crease is faint
  • Your crease is wide and the scar stands out
  • Your double eyelids are uneven
  • Your crease does not suit your face and looks unnatural
  • You have a scar left from epicanthoplasty

What is revision eye surgery?

It is a second surgery to correct an earlier eye surgery you are unhappy with, or to change the line and shape of your crease. We reshape your eyes the way you want.

A revision is worth considering when the crease is too wide, the two sides have become uneven, or the crease has come undone.

A revision needs an accurate read of the cause and a method that fits it. We recommend consulting a specialist with experience in eye surgery.

When a revision can be done

Here is a general guide to timing. It varies from person to person, so please contact us first.

  • About 6 months after the previous surgery, the need for a revision can be judged accurately.
  • If inflammation has occurred, the revision is done after it settles.
  • Depending on your condition, you may need to wait about a year.

Aftercare

Swelling and bruising vary from person to person. To help your recovery, please follow these points.

  • Keep the eye area cool from right after surgery until the swelling goes down.
  • Avoid sleeping face down or on your side. Raising your head on 2–3 soft pillows or cushions eases swelling and pain.
  • Avoid crying a lot, rubbing your eyelids hard or squeezing your eyes shut.
  • Avoid alcohol and smoking for 3–4 weeks, as they slow wound healing.
  • You can wear contact lenses after 2 weeks. Until then, wear glasses or sunglasses.
  • Light makeup is fine after 2 weeks, and eyelash extensions after about 1 month.
  • Avoid baths and saunas for about 3 weeks.

Risks and side effects

Results and recovery vary from person to person. The main risks and side effects are:

  • Swelling and bruising after surgery or treatment
  • Infection or inflammation
  • Asymmetry, or a result that differs from what you pictured
  • Scarring (when an incision is made)
  • Numbness or changes in sensation (often temporary)

If anything concerns you, you can still reach us on WhatsApp after you fly home.

Revision by problem

We correct what happened in your previous surgery

Common problems, and how each one is revised.

Crease set too high

Often the crease was simply designed too high, and sometimes there is also too much fatty tissue. If the crease still looks too high and puffy after the swelling has gone down, a revision is needed.

  • When the crease is high

If the crease is only slightly high, it can be corrected with the incisional method.

If it is much too high, it can still be reshaped in a revision.

  • Sausage eyes

The crease is wide and the eyelid looks puffy.

In mild cases, we reduce the muscle below the crease to make the fold shallower.

Severe cases often overlap with a crease set much too high, and are revised the same way.

Crease too low, or undone

  • When the crease is low

The crease was made too low and became an inner double eyelid once the swelling went down. Depending on your eyelid, we widen or correct the crease with the non-incisional or incisional method.

If the eyelid skin has sagged over time and now covers the crease, we remove some of the sagging skin and recreate the crease. In this case, the surgery is the same as a first double eyelid surgery.

    Double Eyelid Surgery
  • When the crease has come undone

A crease that has come undone can usually be redone with the non-incisional or incisional method.

Distorted crease

Instead of a single line, several lines appear to overlap. A revision is needed if the eyes look sunken and harsh, or if the crease has collapsed and looks unnatural.

  • When there is a triple fold

When we firmly create the crease you want from among the lines, the weaker extra line tends to fade.

In more severe cases, we also reposition the muscle and fat.

  • When the crease is distorted

We design the crease you want, open the skin and reposition the tissue where it belongs.

Unnatural crease

A revision is needed if the crease was made too strong, or if the incision scar is large enough to show you had surgery.

  • When the fold is too deep

We make the fold of the crease shallower.

  • When the incision scar stands out

The scar from the earlier surgery is noticeable. We revise it by re-stitching the scar.

If the scarring is severe, a dermis-fat graft taken from the inner thigh is needed.

Uneven double eyelids

  • When the creases sit at different heights

If the creases on the two eyes sit in different places and look unnatural, a revision is needed.

We match the other side to the crease you prefer, so the two eyes line up.
