Major Causes of Upturned Nose After Rhinoplasty

An upturned nose (a condition where the nasal tip is raised) after rhinoplasty is a common concern among patients.
This issue is not only an aesthetic problem but can also lead to functional complications,
so understanding the causes is essential.
Inflammation and Infection Caused by Implants
The most common cause of an upturned nose is inflammation and infection caused by implants,
especially silicone.
Silicone implants are widely used in rhinoplasty, but due to certain characteristics,
they can lead to an upturned nose.
👃Characteristics of Silicone Implants
Silicone forms a capsule membrane inside the body
If this capsule contracts, the nose is pulled upward
It works under the same principle as capsular contracture after breast augmentation
Silicone implants have a straight shape,
so when surrounded by scar tissue that contracts, they tend to be pulled upward.
As a result, the nasal tip becomes raised, leading to an upturned nose.
Damage to Anatomical Structures
The second major cause is damage to the anatomical structure of the nose, especially the nasal septal cartilage.
👃Classification by Severity of Risk
-Mild upturned nose: When only an implant is used without touching the septal cartilage
-Severe upturned nose: When the septal cartilage is harvested or damaged
The nasal septal cartilage serves as a structural support for the nose.
If this cartilage is harvested or fractured,
the entire nasal structure including the columella (the area below the nose) may collapse,
which can lead to a very severe form of upturned nose.
